Who we are
This website, yphuang.ca, is run by Yu-Ping Huang (“we”, “us”). This policy explains how we handle personal information connected with the website and with the messages you send us, and how we handle patients’ personal health information. The website is not directed to children.
What we collect
- When you visit: our hosting provider receives the technical details every browser sends, such as your IP address, your browser type and the pages you request. They are needed to deliver the website and to protect it from abuse.
- When you contact us: your name, your contact details and what you tell us.
Patient information
Please leave out any patient information when you email, call or message us. These channels are not secure for it. Clinical details go through the secured referral portal. If we receive patient information by mistake, we handle it as described under Patient health information below.
How we use it
We use your information to reply to you, to follow up on your request, to keep a record of our correspondence and to keep the website secure. By contacting us, you agree to this use. You can withdraw your consent at any time by writing to us, and we will then stop using your information unless the law requires us to keep it.
Keeping and protecting it
We keep personal information only as long as we need it or as the law requires, and we protect it with reasonable safeguards. No way of sending information over the internet is completely secure.
Your rights
You can ask to see the personal information we hold about you, and ask us to correct it, by writing to us. If you are not satisfied with our answer, you can contact the Office of the Privacy Commissioner of Canada.
Patient health information
For the radiology services we provide, we are a health information custodian under Ontario’s Personal Health Information Protection Act, 2004 (PHIPA). This section is our public statement of how we handle personal health information.
- What we collect: when a clinician refers a patient, we receive the patient’s identifying details, the images, the clinical question and the relevant history.
- Why: to interpret the images, report on them and communicate with the referring clinician, and for other purposes PHIPA permits, such as billing and meeting our legal and professional obligations.
- Who we disclose it to: the referring clinician and others providing care to the patient, unless the patient tells us not to. Anyone else receives it only with the patient’s consent or where the law permits or requires it.
- How we protect it: with reasonable safeguards, and with access limited to those who need it. Service providers that host our systems act on our behalf and must protect it.
- How long we keep it: as long as the law requires. We then dispose of it securely.
- If it is lost, stolen or misused: we notify the patient and, where required, the Information and Privacy Commissioner of Ontario, as PHIPA requires.
- Access and correction: patients can ask for access to their record, or ask us to correct it, by writing to contact@yphuang.ca.
- Complaints: you can raise a concern with us at contact@yphuang.ca, and you have the right to complain to the Information and Privacy Commissioner of Ontario.
